• One copy of your social security card 
• One copy of your passport or your birth certificate 
• Four character letter (notarized) (Sample Template)
• One copy of your utility bill (even if it's not in the applicant's name)
• One copy of your driver license 
• One passport picture 
• Acknowledgement of Person Agreeing to Safeguard Firearm(s)
• One Copy of Cohabitant (The Cohabitant is for everyone that is 18yrs or older who lives with you)
• Affirmation of Familiarity
• Understanding of Penal Law
• Social Media Handle
• One DMV abstract of LIFETIME driving record (3rd option under your DMV account)

request for the lifetime driving records to be emailed to you once ready: Click here)

• Disposition letters if you were arrested
